As of January 1, 2021 healthcare providers are required to provide clients who do not have insurance, or who are choosing not to use insurance, a Good Faith Estimate of services and inform them of their Federal rights and protections against “surprise billing.”

Frequently Asked Questions (FAQS)

What is a Good Faith Estimate?

Effective January 1, 2022, a ruling went into effect called the "No Surprises Act" which requires licensed healthcare providers to provide a "Good Faith Estimate" regarding fees incurred through accessing Out-of-Network care as a self-paying client.

Under Section 2799B-6 of the Public Health Service Act (PHSA), health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request, or at the time of scheduling health care items and services to receive a "Good Faith Estimate" of expected charges.

This Act requires that we notify you of your federally protected rights to receive a notification when services are rendered by a non- insurance participating provider and provide options to receive care from an In-Network provider, if one is available to take new clients. Goal Driven Counseling is In-Network with several insurance companies; however, every provider does not accept the same insurances.

You have the right to receive a Good Faith Estimate within the following timeframes:

  • If the service is scheduled at least three business days before the appointment date, no later than one business day after the date of scheduling
  • If the service is scheduled at least 10 business days before the appointment date, no later than three business days after the date of scheduling; or
  • If an uninsured or self-paying client requests a Good Faith Estimate (without scheduling the service), no later than three business days after the date of the request.

If you have further questions you may contact us by phone at 855-524-5222 or by email at admin@goaldrivencounseling.com

What insurances do you accept?

We only accept limited insurance carriers in our headquarter in the state of Missouri, all other states are self-pay.

Insurance coverage and co-payment amounts vary depending on your specific plan. Once we have confirmed your appointment request, you will receive an email to complete out insurance form and upload a copy of the font and back of your insurance card. This is necessary so we can verify your insurance, otherwise, you will be required to pay out of pocket for your initial appointment.

Accepted insurance carriers as of 2023:

Cigna

Medicaid

Note, every therapist does not accept the same insurances. Please review individual providers profiles.

What if you don't accept my insurance?

If we do not accept your insurance provider or we don't have a therapist with availability to see your insurance type, you have the option to pay our flat rate and receive a superbill (a detailed receipt that provides all necessary info for insurance reimbursement). With this superbill, you can potentially be reimbursed a percentage of the session fee if you have Out-of-Network benefits.

If you have a presenting concern that potentially fits under our coaching options, you may be able to pay an even lower rate to meet with one of our Goal or Wellness Coaches.

How do I verify if I have Out-of-Network benefits?

You can call your insurance company to inquire so you can make a fully informed decision on how you'd like to proceed with care.

You can ask:

  • Do I have out-of-network mental health benefits?
  • Do I have coverage for telebehavioral health services?
  • What is my deductible and has it been met?
  • How many mental health sessions per calendar year does my insurance plan cover?
  • How much does my plan cover for an out-of-network mental health provider?
  • What’s the process to obtain reimbursement for therapy with an out-of-network provider?
  • What is the coverage amount per therapy session?
  • Is approval required from my primary care physician?
When will I be billed?

You card will be charged the next day following your appointment and a cancellation fee of the full service amount is enforced for a missed appointment without a 24 hr cancellation.

Do you accept HSA cards?

Conveniently, individuals who have Health Savings Accounts (HSA) can pay for counseling services through this pre-taxed account. Entering one of these forms of payment will be required following the Pre-Screen when you request an appointment with one of our counselors.

What's the cancellation policy?

You are expected to provide a 24 hr notice of cancellation directly to your provider. If you fail to provide a 24 hr notice, you will be charged for the missed appointment. Additionally, because of the short-term nature of the Coaching program, missing a week may lead to fewer sessions to meet with your coach. If you miss or cancel more than one Coaching session, or don't follow-up with your coach after a missed session, you may lose your spot in the program.
